CONSENT FOR DATA PROCESSING

Hereby, I provide consent to the controller the Slovak Accreditation Agency for Higher Education, headquartered in Nám. Slobody 6943/11, 811 06 Bratislava – Staré Mesto, Registration Number 52113680 (referred to as “the controller”) to process my personal data in the following scope:

email address, citizenship, age, gender, higher education institution, faculty, study field, study programme, level of study, form of study, status of study at the current level, student’s special needs according to § 100 (2) of Act No. 131/2002 Coll.

for the purposes of participating in the Student Survey – Quarter to Quality Education.

Personal data will be retained for the period of eleven months since the consent is granted. After this period, the data will be disposed of. Personal data you provide are neither subject to profiling nor automatic decision-making.

The controller neither intends to transfer personal data to a third country nor to an international organisation. Providing personal data and consent for processing of personal data is voluntary. You can withdraw your consent at any time by sending a consent withdrawal in writing to the controller’s address. Consent withdrawal enters in force upon its date of service.

Hereby I, the data subject, confirm that the controller informed me about my right to withdraw this consent any time by sending a request to the controller’s email address gdpr@saavs.sk or a request in writing to the controller’s address: The Slovak Accreditation Agency for Higher Education, Nám. Slobody 6943/11, 811 06 Bratislava – Staré Mesto.

By providing consent, your rights as a data subject are not affected and you can exercise them by sending an email to the email address stated above or a regular mail to the controller’s address. As a data subject, you have a right to:

  • request access to your personal data;
  • request rectification of your personal data;
  • request erasure of your personal data;
  • request restriction of the processing of your personal data;
  • object to the processing of your personal data; and
  • request transfer of your personal data.

In case you are not satisfied with the way your request is handled, you have a right to make a complaint at the Office for Personal Data Protection of the Slovak Republic, Hraničná 12, Bratislava 820 07.
